When planning for cheekbone augmentation, I consider the following six key factors:
1. Balance & Facial Harmony – Ensuring the augmented cheekbones complement the patient’s overall facial proportions, avoiding over-projection or asymmetry. The goal is to create a natural and aesthetically pleasing contour that enhances the midface without overpowering other features.
2. Anatomy & Bone Structure – Understanding the patient’s underlying zygomatic structure, soft tissue volume, and existing projection is crucial. I assess whether augmentation should be lateral (zygomatic arch), anterior (malar prominence), or a combination to achieve the desired look.
3. Relationship with the Tear Trough & Infraorbital Region – Proper augmentation should not encroach on the tear trough or create an unnatural transition between the orbital rim and the cheek. Overfilling in the wrong plane can lead to excessive convexity or a “puffy” look under the eyes.
4. Nasolabial Fold Improvement – Cheek augmentation, when performed correctly, provides indirect support to the midface, reducing the appearance of deep nasolabial folds. However, I also evaluate whether additional volume in the deep pyriform space is needed to optimize results.
5. Soft Tissue Dynamics & Aging Considerations – Augmenting the cheekbones must account for long-term changes in soft tissue descent. Over-projecting in younger patients may lead to an unnatural look as they age, while in older patients, strategic lifting of the midface can restore a more youthful contour.
6. Material Selection & Placement Technique – Whether using implants, fat grafting, or fillers, I tailor the approach based on longevity, patient anatomy, and desired outcome. Proper placement in the deep fat compartments or directly over bone ensures a structured yet natural augmentation.
