Understanding Fat in the Body: What Five Pounds Really Looks Like

When people hear that a procedure removes “a few pounds of fat,” it can be difficult to picture what that actually means. Numbers on a scale don’t always translate clearly to physical space inside the body.

That’s why physical models can be so helpful. In the office, we often use a model that represents five pounds of fat, replicated to match both the weight and the volume it occupies beneath the skin. When patients see it in person, the reaction is almost always the same—surprise at how much space even a relatively small amount of fat can take up.

Five pounds of fat may not sound like much numerically, but visually it occupies a significant amount of volume. Seeing it physically helps patients better understand how fat distribution affects body shape and why removing or reducing it can create meaningful contour changes.

How Plastic Surgery Addresses Excess Fat

In plastic surgery, there are several ways to treat excess fat depending on where it’s located, how much is present, and what the patient’s goals are. Generally speaking, there are three primary approaches: injectable treatments, liposuction, and excisional surgery.

Each method is designed for a different situation.

Injectable Fat Dissolution

For very small, localized fat deposits, injectable treatments can sometimes be the best option.

Kybella is an FDA-approved injectable treatment that dissolves fat cells in targeted areas. It is most commonly used beneath the chin to treat submental fullness, sometimes referred to as a “double chin.”

The treatment works by breaking down fat cells, which are then naturally cleared by the body over time. Because it is injectable, it avoids surgery and incisions, making it appropriate for small areas where only a modest amount of fat reduction is needed.

However, injectable fat dissolvers are not designed for larger areas of the body.

Liposuction for Precision Contouring

For moderate amounts of fat, liposuction is often the most effective approach.

Liposuction is a minimally invasive procedure in which fat is removed through small tubes called cannulas. These cannulas are inserted through tiny incisions and allow the surgeon to sculpt and contour specific areas with precision.

Common areas treated with liposuction include:

• Abdomen
• Flanks (love handles)
• Thighs
• Neck
• Arms
• Back

One of the advantages of liposuction is that it allows the surgeon to carefully reshape the body’s contours rather than simply reduce weight. Even relatively small amounts of fat removal can create noticeable changes in silhouette and proportion.

Excisional Surgery for Larger Changes

In some situations, fat reduction alone isn’t enough. When patients also have significant excess skin or tissue, excisional surgery may be the most appropriate solution.

For example, a procedure like Abdominoplasty removes both excess fat and excess skin from the abdomen while tightening the abdominal wall. This is often recommended for patients who have experienced major weight loss or pregnancy-related skin laxity.

Similarly, Breast Reduction removes excess breast tissue, fat, and skin to reshape the breasts and create a more proportional, comfortable result.

These procedures involve surgical removal of tissue and are designed to address more substantial anatomical concerns.

Choosing the Right Approach

Every patient’s anatomy is different, which means there is no single solution for fat reduction or body contouring. The most effective treatment depends on several factors:

• The amount of fat present
• The specific area of the body being treated
• Skin elasticity and quality
• The patient’s aesthetic goals

For very small fat deposits, injectable treatments may be sufficient. For moderate contouring needs, liposuction offers precise sculpting. For larger structural changes involving both fat and skin, excisional surgery may provide the best outcome.

Seeing the Difference

The five-pound fat model is a powerful reminder that body contouring is about volume and shape—not just weight.

Even relatively small amounts of fat can occupy significant space beneath the skin, and removing or reducing that volume can dramatically refine the body’s contours. When patients can see and hold that physical representation, it often makes the process much easier to understand.

In aesthetic surgery, thoughtful technique selection and careful planning are what ultimately create natural, balanced results—tailored to each individual patient’s body and goals.
