Excellent plastic surgery whispers; it doesn't scream. Particularly for facial plastic surgery, beautiful results must keep the natural character of your face and make you look like a younger, more attractive version of yourself.

You lead with your face, and your face is your identity. Having obvious signs of surgery-- such as tightly pulled skin, permanently surprised-looking eyes, and puffy cheeks-- is unattractive and disconcerting.

When I am performing facelift, necklift, and eyelift (blepharoplasty) surgery, these thoughts are always my main concerns. I use the most modern techniques available to keep my patients looking natural.

Here are some of my surgical approaches to achieve the natural look:

  • Create scars that are hidden inside the ears, along pre-existing scars, or under the hairline
  • Lift the substructure of the face (the deep plane SMAS) in a vertical vector to create a beautiful jawline and cheek
  • Gently redrape the skin in a transverse vector to create a smoother, more youthful look, without making it look shiny or tightly pulled
  • Use judicious fat grafting to fill hollows and areas of volume loss

Injectable tear trough filler is an expert technique for improving dark circles and puffy bags under the eyes. I have a special interest in procedures that address the face and eyes, and this procedure is one of my favorite quick fixes.

While blepharoplasty (eyelift surgery) is the gold standard for tired under-eyes, injection of hyaluronic acid based filler (such as Restylane, Juvederm, and Belotero) is a non-surgical alternative.

"Tear trough filler," as the procedure is called," takes 5-15 minutes, involves no incisions, anesthesia, or surgery, and lasts for 1-2 years. It makes a dramatic improvement in dark circles, puffy under-eye bags, and overall tiredness of the eyes.

This is an expert technique, though, so I caution patients to think carefully before choosing a provider. There is a risk of permanent blindness and disfigurement if tear trough filler is done inappropriately.

Liposuction is one of the most popular cosmetic surgery procedures worldwide, and tens of thousands of Americans undergo liposuction each year. With the rapid proliferation of liposuction has come the unfortunate consequence of poorly regulated liposuction, sometimes with unfavorable results.

Most doctors who perform liposuction are NOT board-certified plastic surgeons, so it is important to thoroughly research your doctor in advance of seeking treatment.

While many inexperienced practitioners regard liposuction as an "easy" surgery, getting beautiful, durable results with nice, smooth contours is actually quite challenging. When I am performing liposuction, I make special efforts to ensure that I suction fat in an even, deep plane. I use cross-hatching techniques at a sufficient tissue depth. I use multiple access sites and cannula sizes. And I use tumescent technique.

What this means is that I am able to give patients a smooth, natural reduction in fat that looks even and beautiful from all angles. 

I also perform correction of previous liposuction that was not done to a patient's standards. If you are unhappy with previous liposuction done by another surgeon, your options include repeat liposuction, autologous fat grafting, or both. Sometimes repeat procedures are needed to get perfect results.

In my New York City plastic surgery practice, many patients are interested in Botox (or related neuromodulator injections, Dysport and Xeomin). Not only is Botox the most popular cosmetic procedure in America, but it is also the most effective way to smooth out wrinkles and improve skin texture.

What is the best indication for Botox?

Botox is best used to:

  • smooth out facial wrinkles in the forehead, glabella (between the eyebrows), and crows feet
  • improve the proportions of facial features, such as the arch of the eyebrows, the resting position of a scowling mouth, and a gummy smile
  • soften facial wrinkles while still allowing normal facial expressions

What is the best age to start using Botox?

Botox is best started before wrinkles develop in repose (while the face is at rest). Its main strength is treating dynamic (moving) wrinkles. If you have deep furrows that are present when the face is at rest, you may need an injectable filler like Juvederm, Restylane, Belotero, or Voluma to fill in those creases.

Many of my patients start using Botox as young as their 20s. I, in fact, started using Botox in my late 20s as well. Botox at this age is meant to be preventative-- if you don't ever develop the deep facial wrinkles that come with sun damage, genetics, age, and environmental factors, your face will age more gracefully and beautifully over time.

Will Botox freeze my face?

Not if it's done by someone with a light touch, meticulous microdroplet technique, and thorough understanding of facial anatomy. Well-done Botox will relax your face but preserve your ability to make normal facial expressions, express yourself, and feel normal. I do Botox for many people who work in professions where facial expressions are vitally important-- actors, actresses, psychiatrists, therapists, lawyers, politicians, and parents-- all of whom need to express a range of emotions on a regular basis.
